Certification Forms

Please note that during the certification process some documents requested may not apply depending on a business' structure, licensure, and/or services provided. In instances where requested documentation may not be applicable to a certification application, it is required that the applicant indicate such documents and a detailed reason for exclusion in a cover letter on the company's letter head. 

This list must:
  • Be comprehensive in covering each omitted document.
  • Include an explanation for omission.
  • Be on the official company letterhead.
  • Signed by the applicant.
  • And submitted as the first page in the certification application submitted to NAWRB.
Failure to include this letter may result in significant delays in a certification's examination and potentially harm chances of approval. Any questions should be addressed to info@NAWRB.com.
